What is trauma therapy?

Trauma therapy is for experiences your nervous system has not finished processing. A single event such as an assault, an accident or a sudden loss. Or something long-running, such as growing up in a frightening home. The effects show up as flashbacks, nightmares, feeling constantly on alert, going numb, avoiding reminders, or a sense of being cut off from yourself.

Several approaches have good evidence behind them. EMDR uses side-to-side eye movements or tapping while you hold the memory in mind, which appears to help the brain file it as past rather than present. Trauma-focused CBT works on the beliefs the event left behind. Somatic approaches work with what the body is holding. Psychodynamic and relational therapy looks at how the experience shaped the way you relate to people now. A good therapist will match the approach to you rather than the other way round.

What to expect from trauma therapy

Good trauma work does not start with the trauma. The first few sessions are usually about stability: how you cope now, what helps you settle, what you do when the feelings become too much. Skipping this stage is how people end up flooded and drop out. If a therapist wants to go straight to the worst memory in session one, be cautious.

When you do turn towards the memory, it is done in manageable pieces and at a pace you set. With EMDR, the sessions are structured and the therapist checks in constantly on how distressing the memory feels. Progress is uneven. Many people feel worse for a short period before things settle, which is worth knowing in advance so it does not read as failure. Most trauma work is measured in months rather than weeks.

Do I have to describe what happened in detail?

How many sessions does trauma therapy take?

A single-incident trauma sometimes resolves in eight to twelve sessions. Trauma running through childhood usually takes considerably longer.

What if I do not have a PTSD diagnosis?

You do not need one. Plenty of people carry the effects of frightening or neglectful experiences without meeting the criteria for a diagnosis, and therapy is still appropriate.
